I am not available weekends or after 5PM, so please do not request them. My number is: 937-304-4301. Messages received outside of my normal business hours will be returned the following business day.

First time clients - to save time, please print and fill out these medical history/release forms and bring them with you. If you forget them, we will have copies in the office. If you need to fill them out in the office, plan to arrive 10 minutes early so that the paperwork time will not take time away from your massage.

Please be aware - if you are late for your appointment, you will still be charged the full price for the massage; we will try to accommodate you by extending the massage to match your desired massage length, but we may be unable to do so due to other scheduled appointments.
